Does the QR code actually open the tagged website?

Yes. The QR code contains the complete generated campaign URL. Scanning it opens that URL, including its UTM parameters.

Does creating a UTM link automatically install GA4?

No. The destination website still needs a working GA4 implementation. A tagged link supplies campaign information; it does not install analytics.

Should Google Ads links use manual UTMs?

Google recommends connected ad-platform integrations and auto-tagging where available. Manual UTMs may still be required for another measurement system, but the setup should be reviewed before combining them.
